Dota 2 patch 7.38 revamped the neutral item system in the game. Instead of picking one item out of five, players now select a base item from four choices and apply an enhancement to it. The base item provides an ability, while enhancements provide specific stat bonuses.

In this article, we will go through the best option to pick in each neutral tier list and why they are the best choice for supports in Dota 2 patch 7.38.

Tier 1 - Trusty Shovel

At tier 1, Trusty Shovel is the best choice. Anything you dig up is useful early game. Mangos and Salves help keep you and allies ready to fight, while Bounty Runes are a nice injection of gold for the whole team, especially with the decreased frequency of their spawn rate. Kobolds provide a GPM aura for the team and can be used to tank neutrals and scout ahead for you.

If you don't get Shovel, the second best option is Mana Draught. The extra mana regeneration is extremely useful early game and the low cooldown in water is a nice touch.

Tier 2 - Essence Ring

A classic returns and it's as good as it was before. Essence Ring provides an emergency health boost, useful for supports who are diving in or being attacked. It does have a relatively high mana cost but it is still the best support neutral item in Dota 2 patch 7.38 for tier 2.

If you don't get Ring, we recommend Gossamer Cape to dodge big physical attacks. Some pros keep Cape even until tier 3 and 4. It's a very strong option but reliant on facing strong enemy right clickers.

Tier 3-  Gale Guard

No other item provides as much durability in tier 3 as Gale Guard. You are practically immune to slows while having a 400 damage barrier for 10 seconds. This guide has focused on defensive choices so far and the truth is as a support, staying alive is your top priority and Gale Guard helps in that area immensely.

If you don't get Gale Guard as an option, we recommend Whisper of Dread to dish out some damage, or Ninja Gear for the free (though short-lived) smokes.

Tier 4 - Ogre Seal Totem

Tier 4 provides the perfect combination of offense and defense in one item. Ogre Seal Totem deals 250 damage per bounce and slows enemies while allowing the caster to move forward. It is fantastic to use when escaping and silenced, or when you're melee and want to dish out more damage. It is the best item in this tier by far.

If you are unlucky, then take Ceremonial Robe for melee supports and Monocle for ranged spell casters. Both are solid options and well worth taking.

Tier 5 - Minotaur Horn

Few supports can afford to buy a BKB, so having one as a neutral item provides incredible survivability in late game situations. It does require actually using the item, but when pulled off, it's a great ability to have.

Magic Lamp, Spider Legs, and Book of the Dead are all good choices as well. Choose which you believe best fits the situation. Fallen Sky is  good too, but it's the most offensive option we can recommend in tier 5.
